The idea
Use das Formular for the nominative singular, das Formular for the accusative, dem Formular for the dative, and des Formulars for the genitive. In the dative plural use den Formularen.

Meaning and useful combinations
Formular is neuter and forms Formulare. Ausfüllen means completing its fields; unterschreiben means signing it.
