The idea
Zum Gespräch (zu dem) names the occasion. Gelungen (successful) is the participle of gelingen and works as an adjective: eine gelungene Aufgabe. Ordnen nach plus dative sorts things by a criterion: nach Aufgabe und Ergebnis. “Über etwas sprechen” takes the accusative: über Ihre Entwicklung. In many German workplaces, feedback talks are fairly structured, and concrete examples make them more useful for both sides.
Other ways to say it
“Wie kann ich mich auf das Gespräch vorbereiten?” – Open question; sich vorbereiten auf takes the accusative.
“Soll ich etwas Schriftliches mitbringen?” – Asks about the format of your notes.
“Ich habe zwei Beispiele aus den letzten Wochen vorbereitet.” – States your preparation at the start of the meeting.

Watch out
“Welche Beispiele soll ich zum Feedbackgespräch bringen mit?” → “Welche Beispiele soll ich zum Feedbackgespräch mitbringen?”: After a modal verb the separable verb stays in one piece as an infinitive. Splitting it, as in a main clause without a modal, puts mit in the wrong place.
