The idea
Das Protokoll is the record of a meeting. “Jemandem etwas zuordnen” means to assign something to someone; in the passive the person stays in the dative: “wurde mir … zugeordnet”. A contrast with two short main clauses separated by a comma makes the correction unmistakable. “Etwas unverändert lassen” means to leave it unchanged. Anpassen is separable: “Ich passe … an.”
Other ways to say it
“Im Protokoll hat sich ein kleiner Fehler eingeschlichen.” – Polite and indirect; sich einschleichen means to creep in.
“Bei Punkt 3 bin ich als Versender eingetragen, das stimmt nicht.” – Neutral; points to the exact line.
“Kannst du bei mir „prüfen“ statt „versenden“ eintragen?” – Informal with du; asks for the exact change.

Watch out
“Im Protokoll wurde ich die falsche Aufgabe zugeordnet.” → “Im Protokoll wurde mir die falsche Aufgabe zugeordnet.”: Only the accusative object becomes the subject in a German passive. The person stays in the dative (mir), unlike English “I was assigned”.
