The idea

Heißen is used for names: Sie heißt Selin Aras. Wie lautet …? is a formal way to ask what something is called or says. Mütterlicherseits and väterlicherseits mean on the mother’s or father’s side. Vorlesen (read aloud) is separable and takes a dative person: Lesen Sie mir die Ziffern vor. Der Name is a weak noun, so its accusative is den Namen.

Other ways to say it

“Ich buchstabiere den Nachnamen: A wie Anton, R wie Richard, A wie Anton, S wie Samuel.” – Shows how to spell with the German spelling alphabet.

“Als zweiten Kontakt möchte ich ihre Tante angeben.” – Neutral way to name the second contact on a form.

Watch out

“Danach bestätigen wir Name und Nummer zusammen.” → “Danach bestätigen wir Namen und Nummer zusammen.”: Name belongs to the weak masculine nouns that add -n outside the nominative. As the object of bestätigen it must be Namen.
