The idea
Fragen nach + dative asks about something: nach meiner Wohnadresse. In an oder-question the second item shares the preposition: nach … oder (nach) der Postanschrift. Abweichen von + dative (to differ from) is separable. Bei mir means in my case. Einzeln (individually) and zusätzlich (additionally) are adverbs.

Watch out
“Diese weicht bei mir von die Wohnadresse ab.” → “Diese weicht bei mir von der Wohnadresse ab.”: Von always takes the dative, so die Wohnadresse becomes der Wohnadresse. Abweichen von is a fixed combination worth learning together.
