The idea
Der Beleg and die Quittung both mean receipt; Quittung is the everyday word in taxis. Schreiben auf takes the accusative because the writing goes onto the paper: auf den Beleg. Start und Ziel need no article in a list. Past events are told with the Präteritum in short reports: begann (from beginnen), war. “Darauf” replaces “auf den Beleg” in “Soll auch die Uhrzeit darauf?”.
Other ways to say it
“Ich bräuchte bitte eine Quittung.” – Short and polite; the standard request at the end of a ride.
“Können Sie die Strecke bitte auf der Quittung vermerken?” – More formal; vermerken means to note down.

Watch out
“Könnten Sie Start und Ziel auf dem Beleg schreiben?” → “Könnten Sie Start und Ziel auf den Beleg schreiben?”: With schreiben the text moves onto the paper, so auf takes the accusative den. On the receipt as a fixed place, as in „Auf dem Beleg steht …“, it is dative.
