The idea
A cancellation is formal: Sie, a Betreff such as “Kündigung Abonnement, Kundennummer …”, “Sehr geehrte Damen und Herren”. “Hiermit” opens an official declaration. Many people write “Hiermit kündige ich …” for the clearest wording. Then ask for the end date, any missing details (“nennen Sie diese bitte konkret”) and written confirmation with the date. Do not add reasons unless you want to. Sign off with “Mit freundlichen Grüßen” followed by your first name and surname.
Other ways to say it
“Hiermit kündige ich mein Abonnement zum nächstmöglichen Zeitpunkt.” – Standard formal wording; leaves no doubt about the intention.
“Bitte bestätigen Sie mir die Kündigung und das Vertragsende.” – Formal request that names both points to confirm.

Watch out
“Hiermit kündige ich mich von meinem Abonnement.” → “Hiermit kündige ich mein Abonnement.”: Kündigen takes the contract as a direct accusative object. Learners copy “sich abmelden von” and add mich and von, which is wrong here.
