The idea
A good Übergabe answers four questions: what is prepared, where it is, what is urgent, and who is responsible. Abwesenheit is the neutral word for any absence and avoids private details such as illness. Name the stand-in with title and surname (Frau Nguyen) in mixed teams, and point people to her with direkt mit ihr. Vorgang is office German for a case or matter. Many people also set an out-of-office reply (Abwesenheitsnotiz) that names the same contact.
Other ways to say it
“Während meines Urlaubs vertritt mich Frau Nguyen.” – Standard formula; vertreten means to stand in for someone.
“Ab Montag bin ich zwei Wochen nicht im Büro.” – Neutral opening that gives the dates first.

Watch out
“Während meinem Urlaub vertritt mich Frau Nguyen.” → “Während meines Urlaubs vertritt mich Frau Nguyen.”: In written standard German, während takes the genitive: meines Urlaubs. The dative is common in casual speech, but a handover email to the team should use the genitive.
