Fill in a simple classroom form for an invented person. The examples show both sentences about writing and the short entries that go into boxes.

The idea

In separate boxes, ad asks for the given name and soyad for the surname; ad soyad asks for both. Doğum tarihi means date of birth, adres means address and il means province. Enter the requested information without turning it into a sentence. This practice form uses day.month.year for dates; follow the format shown on the form. In the address, No identifies the building number and Daire the flat.

Watch out

Do not copy the location ending from İzmir’de yaşıyorum into the province box: write İzmir. Likewise, Adım Ece is a sentence, while Ece is the entry under Ad. Keep the dotless ı in Yılmaz.

Try it yourself

  1. The fictional person is Ece Yılmaz. Fill only the given-name box: Ad: ___.

    Show answer or example answer

    Ece

Ece is the given name in the supplied identity; Yılmaz belongs in the other box. Adding Adım would turn the entry into a sentence.

  2. For Ece Yılmaz, fill only the surname box: Soyad: ___.

    Show answer or example answer

    Yılmaz

Yılmaz identifies the family, so it belongs under Soyad. Its second letter is dotless ı, which must stay in the written name.

  3. Write 14 September 2002 in the date-of-birth box, using day.month.year with two digits for day and month.

    Show answer or example answer

    14.09.2002

14.09.2002: the day is 14, September is month 09 and 2002 is the year. The zero gives the two-digit month requested by this exercise.

  4. Choose the province entry: İl: ___ (İzmir or İzmir’de).

    Show answer or example answer

    İzmir

İzmir names the province. İzmir’de means “in İzmir”, so it answers a location question instead of supplying the name alone.
