Use ようにする when someone makes a conscious effort to do or avoid an action.

The idea

Attach ようにする to the dictionary form or the plain negative ない form. ようにします can announce a decision; ようにしています describes an ongoing effort or habit. ようにしてください asks someone to make that effort.

Watch out

Do not use the polite ます form before ようにする. Compare ようになる, which describes a change in state or ability, not the deliberate effort itself.
