A lost card is easier to report when you can explain what happened and ask for the next step.
The idea
使ったのは explains when or where you last used it; 利用を止めてください requests a suspension. 再発行 means issuing a replacement.
Examples
I lost my bank card.
I last used it yesterday at the station.
I do not know where I lost it.
Please suspend the card.
Has it been used by someone else?
How can I get a replacement card?

Watch out
なくしました says you lost it; it does not establish theft. If you do not know where it happened, say わかりません rather than guessing.
