Use a polite exchange to cancel an appointment and, if needed, arrange another date.

The idea

State the date, time and name to identify the booking. 行けなくなりました means circumstances have changed and you can no longer go. 予約をキャンセルしたいです states your wish to cancel; 別の日に予約してもいいですか asks about booking another day.
