Give the manager the current facts, the possible impact and a concrete next step.

The idea

Use 可能性があります to describe a possible outcome; おそれがあります highlights an undesirable one. 〜なければ sets a condition. End with ご判断いただけますか to request a decision politely.

Examples

I have something I would like to discuss with you.
We have not yet received a response from the client.
At this rate, we may not meet the delivery deadline.
If there is no response by next week, the work may be delayed.
I suggest that we follow up with them.
Could you decide how we should respond?

Watch out

A risk is not a confirmed delay. Avoid presenting uncertainty as a fact, and give the condition or evidence behind it.
