Identify the equipment, describe what happens and say when the problem began.
The idea
Use 壊れました to report a breakdown. The pattern 電源を入れても means “even when I turn it on”; 確認していただけますか politely asks someone to check.
Examples
Excuse me, the printer has broken.
Even when I turn it on, it does not work.
An error is showing on the screen.
I have been unable to use it since this morning. I am having trouble.
Could you check it, please? Thank you.
May I use another printer in the meantime?

Watch out
A screen error is an observed symptom, not proof of the cause. Describe what you see rather than guessing why it broke.
