When a service problem remains unresolved, explain what happened and ask for a concrete next step.

The idea

まだ解決していません reports that the problem remains unresolved. いただけますか softens a request; ご相談いただけますか asks the employee to consult someone, not necessarily to transfer you. いつまでに asks for a deadline for the reply.

Examples

I requested a repair last week, but the problem is still unresolved.
The reference number for my previous inquiry is 1234.
I tried the method you suggested, but it did not improve the situation.
Could you please consult the person in charge?
By when can you let me know the plan for handling this?
If that is difficult, please also consider a refund.

Watch out

Do not confuse asking when you will hear the plan with asking when the repair will be completed. A refund is a request to consider, not a guaranteed outcome.
