At the library counter, say you would like a card and ask what you need.

The idea

作りたいです expresses a polite wish to obtain a card. 作るには asks what is needed for that purpose; てください requests an action.

Examples

Excuse me, I would like to get a library card.
What do I need to get a card?
Please show me something that confirms your address.
Please write your name and address on this application form.
Should I write my phone number here too?
When can I start using the card?

Watch out

Requirements differ by library. A document confirming an address is an example, not a universal rule; ask the staff what they accept.
