Read an insurance notice by separating the institution, the payment and the cover. SGK stands for Sosyal Güvenlik Kurumu, the Social Security Institution. The examples are invented language practice, not decisions about anyone’s insurance status.
The idea
Read the whole expression: sigorta means insurance, while sigortalı describes an insured person. Genel sağlık sigortası (GSS) names general health insurance; özel sağlık sigortası names private health insurance. In a payment notice, prim is an insurance premium or contribution, prim borcu an unpaid contribution, and katılım payı a patient copayment. In a policy, teminat concerns cover; kapsam dışında places something outside its scope. Ask about a particular expense with Bu masrafı sigorta karşılıyor mu? Here karşılamak means to cover a cost.

Watch out
Do not read prim borcu as a charge for a medical visit: the debt concerns insurance contributions. Likewise, Sigortam var says that you have insurance; it does not say that every expense is covered. A private policy’s cover has terms and limits.
